Your Individual Care Plan

Beyond the above basic services, we will meet with you, your family, and your physician to create a care plan specific to your needs.  Care plans can be as basic or complex as they need to be, and assistance can be added or taken away at any time at your discretion. 
Services available to you include:

v      Medication management by licensed nurses (including diabetic management, oxygen needs, wound care, and catheter/ostomy care)

v      Bathing/Showering assistance                                 

v      Dressing/Grooming

v      Assistance using the restroom

v      Incontinence Care

v      Medical appointment coordination

v      Wheelchair/ambulatory escorts throughout the community

v      Reminders/orientation due to memory impairment

v      Modified meals for specified dietary requirements

v      Nighttime safety checks

v      Hospice care

 Resident Transportation

Transportation is included in residency at the community.  Residents go on weekly shopping trips, weekly fun outtings, and can arrange for physician and/or personal appointments (banking, etc.).
